Your child's Bar or Bat Mitzvah is a very special time for your family. We are excited to share this simcha with you. In order to help the Temple staff ensure the day goes smoothly, please complete this form. The Temple is allergy aware and as such we do not serve foods with nuts. Please let us know your child's favorite treats and if he/she has any food allergies so that we can make sure that there are items served at the Oneg Shabbat that he/she can enjoy. We would also like to know if you need any gluten free or sugar free items at the Oneg for your guests.

Bus Information - Heading to Celebration*
Will you be having a bus(es) coming to the Temple directly after your Service? If yes, please let your bus company know that they should park upstairs by the Atrium doors.
Kindly let us know if your bus(es) will be returning to the Temple for children to be picked up. If the answer is yes, please provide us with an approximate time the bus will be at the Temple. The returning bus(es) should park upstairs by the Atrium doors.
